Hlavným hrdinom je Garp, človek s obyčajnými túžbami, ktorého život ovplyvní už jeho počatie. Jeho matka, rozhodnutá mať dieťa bez partnera, si za otca vyberie nevyliečiteľne chorého vojaka. Tento vzťah formuje Garpov život a jeho osobnosť. Postavy v románe, vrátane Garpovej matky, manželky Helen a ich synov, sú realistické a ich činy majú silný dopad. Irving podobne ako v iných románoch excelentne zaznamenáva nuansy bizarného rodinného života súrodencov Berryovcov – homosexuála Franka, Franny a Johna spojených incestným vzťahov, trpaslíčky Lilly s literárnym talentom a kurióznej postavičky Vajca.

Irvingov trinásty román je rozprávanie o túžbe, o tajomstvách, o hľadaní sexuálnej identity a o nenaplnenej láske – manifest inakosti a ďalší autorov bildungsromán, ktorého ústrednou postavou a zároveň rozprávačom je Billy Abott, úspešný takmer sedemdesiatročný spisovateľ. Pod vplyvom jednej zo svojich lások, slečny Fosterovej, knihovníčky v miestnej knižnici, ktorá má tiež svoje veľké tajomstvo, Billy sexuálne precitne a užíva si naplno svoj život.
