Interest in urinary enzymology surged three decades ago with findings that urinary enzymes are elevated in kidney and urinary tract diseases. While over 40 hydrolases, oxidoreductases, transferases, and lyases have been identified in human and animal urine, only about ten are currently used as diagnostic indicators. Advances in understanding the quantitative distribution of enzymes in nephron anatomy and function have enabled the association of urinary enzyme activity patterns with physiological and pathophysiological nephron functions. However, confidence in the diagnostic value of urinary enzymes varies among clinicians and scientists. This discrepancy may stem from the greater variability in urinary enzyme data compared to the more stable data seen in blood plasma enzyme diagnostics. Unlike plasma enzymes, which benefit from a protective environment, urinary enzymes face challenges such as a hostile environment with little protective protein, fluctuating pH, volume, metabolite and salt concentrations, and varying enzyme inhibitor levels. Recent methodological advances allow for better control of these factors, highlighting the importance of standardizing urine collection periods and preanalytical treatment alongside optimizing assay methods.
